# Every medical record.
None of the follow-up.

Tavrn requests, follows up and delivers records, bills and imaging from any US provider into the systems you already use.

## Your work can’t move until the record does.

Every health-data network and portal combined returns about 63% of a patient’s visits. The rest sits in fax queues, portals and records departments that nobody calls.

- 100,000+ provider requests processed
- 100% of providers chased
- Every 48h automated follow-up

## A single process for medical record retrieval

1. **Request** — A new request starts in the Tavrn platform, from Clio, SharePoint, Smokeball, Litify, SmartAdvocate, Dropbox, or from the API.
2. **Track** — A status card with one row per provider: records received, confirming receipt with a provider-stated date, action required to approve a fee, and the next follow-up in 48 hours.
3. **Deliver** — A folder of checked documents filed by provider and date of service, with record type, page count and file type for each file.

### Law Firms

- Start the request from the matter in Litify, SmartAdvocate, Clio, Smokeball, SharePoint, Dropbox, or the API. One signed HIPAA authorization covers every provider; Tavrn prepares each provider’s release.
- Every provider has its own status, stated date, and contact history. Tavrn follows up every 48 hours by fax, portal, mail, email, or phone, and flags fee approvals as action required.
- Checked records land in the matter, with provider fees posted as case costs. Tavrn also pulls the accident report and verifies coverage with each carrier; chronologies and demands build from the file.

### Hospitals & Clinics

- Request a patient’s outside records from one screen, naming each hospital, clinic, imaging center, and lab. One HIPAA authorization covers every provider on the list.
- Tavrn asks networks and portals first, then faxes, calls, or mails the providers they do not reach, following up every 48 hours. You see each provider’s status and contact history.
- Notes, DICOM images, pathology reports, and labs arrive in one folder per patient, by provider and date of service, checked against the request. HIM files them to the chart.

### Health Tech

- Order records from inside your product. One request per provider under the patient’s signed authorization, sent through the REST API or uploaded as a spreadsheet.
- A webhook fires on every status change, so your product always knows where each provider stands. Tavrn follows up with every open provider every 48 hours until records arrive.
- Documents arrive as each provider responds: notes, imaging with DICOM, pathology reports, labs, and bills, filed by provider and date of service. Download by API or SFTP into your pipeline.

### Agents

- Create one retrieval request per provider, with the patient’s signed authorization attached, and keep the request ID. Agents start at tavrn.ai/agents.md.
- Providers answer on their own schedule. A webhook fires on every status change, or poll the request, while Tavrn follows up with every open provider every 48 hours.
- Each file is checked against the request: dates of service, record types, pages. Download documents with the same key, with record type, page count, and file type attached.

## FAQ

## What types of records can Tavrn retrieve?

Medical records, billing records, radiology images, accident reports and insurance coverage from any US provider — plus medical chronologies and demand letters built from the record.

## Which providers can Tavrn reach?

Around 425,000 provider organizations across all 50 states and DC: the health-data networks plus the long tail that only answers fax, portal, mail, email or phone.

## How long does retrieval take?

Requests go to the provider the same day. Every open request is followed up automatically every 48 hours, and you see per-provider status and provider-stated dates live.

## Does Tavrn replace our existing system?

No. Start requests from Litify, SmartAdvocate, Clio, Smokeball, SharePoint or Dropbox and records land back where you work; anything else connects over the API and webhooks.

## How long does it take to get started?

Requests can start the day your team gets access to the Tavrn app. Litify, SmartAdvocate, Clio, Smokeball, SharePoint and Dropbox connect during onboarding without engineering work on your side; API keys are issued in the Tavrn dashboard after a 30-minute call.

# Medical records retrieved, chased, paid and filed in your CMS

Tavrn requests treatment records and itemized bills from every provider on the matter, follows up by fax, portal and phone until they arrive, pays the provider invoices under your approval limit and delivers everything into Litify, SmartAdvocate, Clio or Smokeball. Built for personal injury, mass tort, medical malpractice and workers’ comp firms.

## Retrieval at a scale one firm never reaches

- **100,000+** Completed record requests
- **2–3 weeks** Most records back, request to delivery
- **500+** Organizations retrieving with Tavrn

## Built for every plaintiff practice

### Personal injury

Treatment records and itemized bills from every provider the client saw — ER, imaging, specialists, therapy — are the demand. Nothing goes to the adjuster until the last one is in.

- **Demand-ready records** — Treatment and billing records from every provider, with live status on each.
- **Prior treatment** — Pre-suit and litigation records, including prior treatment, on the same authorization.
- **Follow-up every 48 hours** — Demand deadlines don’t wait on a fax queue.
- **Filed to the matter** — Records and bills land in Litify, SmartAdvocate, Clio or Smokeball, with fees posted as case costs where supported.

- [Landver Law: 6x demand output](https://www.tavrn.ai/customer-stories/landver)
- [Desert Injury Law: a multi-state PI operation](https://www.tavrn.ai/customer-stories/desert-injury)

### Mass tort

Hundreds or thousands of claimants, each with their own providers, each needing proof-of-use and injury records before the census or bellwether deadline.

- **Bulk ordering** — One request per claimant, per provider, by spreadsheet or the API.
- **Census-ready status** — Per-claimant, per-provider status and history, exportable for the census.
- **Proof of use and injury** — Pharmacy and treatment records, chased at 100% of providers.
- **Back to each claimant** — Records delivered to each claimant’s matter.

- [Paul LLP: retrieval across hundreds of facilities](https://www.tavrn.ai/customer-stories/paul)
- [The Levensten Law Firm: mass tort discovery](https://www.tavrn.ai/customer-stories/levensten)

### Medical malpractice

The complete chart from the hospital and every treating physician, plus imaging, so your expert reviews the same record the defense will.

- **Complete charts** — Requested provider by provider and checked against the request for anything missing.
- **Imaging and film** — Requested alongside the chart.
- **Expert review sets** — Provider-specific record sets for expert review.
- **Outreach Activity Report** — Every request, contact and delivery logged.

### Workers’ comp

Records and bills from the treating physicians and facilities on the claim, with updates as treatment continues, for the claim file and the hearing.

- **Claim records and bills** — From the treating physicians and facilities on the claim.
- **Ongoing treatment** — Updates requested as the claim develops.
- **Chronology** — History, objective findings and MMI dates pulled from the record.
- **Into your CMS** — Records delivered into your case management system.

- [Martay Law Office: 65% more case filings](https://www.tavrn.ai/customer-stories/martay)

### SSDI

Every treating provider on the disability file — primary care, specialists, mental health, hospitals — gathered from one authorization and assembled for the SSA submission.

- **One authorization** — Every treating provider requested from a single authorization.
- **Ready to submit** — The complete file, organized by provider and date of service.
- **Follow-up every 48 hours** — Automated follow-up with live status on every provider.
- **Fees at exact cost** — Provider fees passed through at exact cost.

## Every demand waits on the slowest provider

### Three weeks pass and nobody knows where the request stands

A paralegal sends the authorization, then calls the records department whenever there is a gap in the day. Hospital portals, fax confirmations, wrong fax numbers and a provider that wants its invoice paid before it will release a page.

**37%** of a patient’s record sits outside every network and portal combined

### Cases sit, clients call, and the fees land on the wrong matter

Nothing goes to the adjuster until the last chart and itemized bill are in. Meanwhile provider invoices, clearinghouse fees and re-requests pile up with nothing tying them back to the case.

### Every provider chased, every contact logged, every fee on the matter

Tavrn sends the request, confirms receipt, follows up by fax, portal and phone until the records and bills arrive, pays the provider under your approval limit and files everything to the matter in your case management system.

**100%** of providers on a request followed up until they produce or confirm no records

## How Tavrn speeds up your records team

### Records filed straight to your CMS

Records, bills and invoice PDFs land on the matter in Litify, SmartAdvocate, Clio or Smokeball, with no re-keying.

### Follow-ups handled for you

Every open request is confirmed and chased by fax, portal or phone until the provider produces.

### Every delivery checked

Each delivery is checked against the request, and missing pages go back to the provider automatically.

### Provider invoices paid for you

Fees are paid at exact cost under the approval threshold you set, then posted to the matter as case costs.

## Everything a case needs.

### Complete

Every network and portal, plus the long tail

- 425,000 provider organizations, all 50 states + DC
- Correct custodian and provider’s own authorization form, resolved for you
- Fax, portal, mail, email, or phone — whatever they respond to

### Chased until it arrives

Receipt confirmed, then followed up by fax, portal and phone

- Each request goes to the custodian the way that provider actually responds: fax, portal, mail, email or phone.
- Receipt is confirmed with the records department, so a lost fax is found in days, not at the deadline.
- Open requests are followed up automatically, including by phone during the provider’s business hours, until records arrive.
- A provider that stalls is escalated to Tavrn’s retrieval team, who work the request by hand.
- Every contact is logged with date, channel and outcome, exportable as the Outreach Activity Report.
- A provider with nothing on file returns a no-records statement, documented on the request.

### Records that hold up

The hard requests, handled as part of the request

- Certified copies and custodian affidavits requested with the record, notarized where the court needs it.
- Every delivery checked against the request. Missing pages go back to the provider; a statement of no records is documented.
- A deposition or trial date on the case moves its requests to the front of the queue.
- Imaging and films, pharmacy, itemized bills and full charts, each requested the way that custodian requires. Mailed discs ingested.
- Patient-portal retrieval when the client’s own portal is the fastest route, by secure text to the client.
- Continued care: re-request the same provider for new dates of service, with duplicates detected.
- Clearinghouses — Datavant/Ciox, MRO, Verisma, Sharecare — handled on their own portals and payment rails.
- Delivered searchable (OCR), with a cover page, organised by provider and date of service.

### The whole case file, not just the chart

Insurance coverage discovery and accident reports as products, requested the same way

- Coverage discovery per matter: BI, PD, PIP/MedPay, UM/UIM lanes, each tracked to a written answer.
- Letters of representation and disclosure requests sent to each carrier, followed by phone.
- Written policy limits collected into one coverage packet for the demand.
- Accident reports located, purchased and delivered to the matter.

### Fast & visible

Live status on every provider

- Per-provider status, provider-stated dates, and full history on every request
- Outreach Activity Report: every contact with the provider, exportable

### Every provider fee, visible

Provider fees paid for you, passed through at cost and tied to the matter

- Provider and clearinghouse fees are paid by Tavrn and passed through to the firm. Your contract sets the pass-through terms.
- Fees above the approval limit you set wait for a yes. Everything under it is paid so the records keep moving.
- Disputed provider invoices, such as per-page charges above the state rate, are challenged for you.
- One consolidated Tavrn invoice, by card or ACH, with every provider fee tied to its matter.
- No separate rush fee: a deposition or trial date on the case moves its requests to the front of the queue.

### Every fee on the ledger

Provider fees and Tavrn fees posted to the matter as native expenses

- Litify Expenses, SmartAdvocate Disbursements and Clio expense entries, created from the request. No re-keying.
- Provider fee and Tavrn fee kept separate, each with its own expense type, vendor and recoverable flag.
- Provider invoice and Tavrn invoice PDFs filed to the matter next to the records.
- A Case Costs page per matter: credits used, provider fees and total, ready for the settlement statement.
- QuickBooks Online: fees posted as bills or purchases against the client, vendors kept in sync.

## Use Tavrn with your existing systems

Works with the systems your team already uses.

- Clio
- Dropbox
- SmartAdvocate
- Litify
- Smokeball
- SharePoint
- Custom integration

## The same records, already working

- **Chronologies and demands** — Records delivered through Tavrn can feed a cited medical chronology and a demand drafted on your firm’s template, reviewed before it reaches you.
- **Case costs** — Provider invoices paid at cost under your approval limit, then posted to the matter as expenses with the invoice PDF filed beside the records.
- **CMS writeback** — Records, bills and invoice PDFs filed to the matter in Litify, SmartAdvocate, Clio or Smokeball, with status and expenses written back where the system supports it.
- **Coverage and crash reports** — Letters of representation and disclosure requests to every carrier, verbal and written limits in one coverage packet, and the accident report purchased and filed.

## FAQ

## How long does retrieval really take?

Most records are back within two to three weeks of the request going out. Each provider’s stated date shows on the request, and every open request is followed up until the records arrive.

## How do we know the records are complete and correct?

Every delivery is checked against the request: pages, date range and record types. Anything missing goes back to the provider, and a provider with nothing on file returns a no-records statement. Each contact with the provider is logged, so you can see exactly what was asked for and when.

## Another vendor quotes a lower price per request. Why Tavrn?

Provider fees pass through at cost under an approval limit you set, there is no rush fee, incomplete productions go back to the provider, and fees post to the matter as case costs. No paralegal hours on follow-up or invoice reconciliation.

## Do we have to commit before we have seen it work?

No. Most firms start with a pilot on live requests from their own matters, with the same follow-up and visibility as a full account, and decide on an agreement when the results are in.

## Does Tavrn work with our case management system?

Litify, SmartAdvocate, Clio, Smokeball, SharePoint and Dropbox are supported today, with an API for anything else. Records and invoice PDFs are filed to the matter; Litify, SmartAdvocate and Clio also receive status and expenses.

## We have an in-house records team. Does this replace them?

It takes the chasing off their desk. Your team still decides what to request and reviews what comes back; Tavrn handles the sending, the confirmation calls, the follow-up, the provider invoices and the filing.

## We already use an AI tool for chronologies and demands. Is Tavrn redundant?

No. Those tools start when the records are in hand; Tavrn is how they get there. Records retrieved through Tavrn can be used with your existing tools, or with Tavrn’s own chronologies and demands.

## Is Tavrn for a small firm?

Yes. A solo practitioner with a few matters a month sends requests one at a time from the app or their case management system. Larger firms and mass tort dockets use bulk ordering and the API.

## Who pays the provider?

Tavrn does, including on clearinghouse portals such as Datavant/Ciox, MRO, Verisma and Sharecare, so records are released without your staff cutting checks. The fee is passed through on your Tavrn invoice and posted to the matter.

## Which providers can Tavrn reach?

Around 425,000 provider organizations across all 50 states and DC: the health-data networks plus the long tail that only answers fax, portal, mail, email or phone.

## Does Tavrn replace our existing system?

No. Start requests from the Tavrn app or connect your own system over the API and webhooks; records land back where you work.

## How long does it take to get started?

Requests can start the day your team gets access to the Tavrn app; API keys are issued in the Tavrn dashboard after a 30-minute call.

## Do we need to use a separate portal?

No. Start requests from the Tavrn app or through the API, and records land back where you already work.

## Can Tavrn get certified copies and custodian affidavits?

Yes. Ask for a certification or affidavit on the request and it goes to the custodian with the record request. Notarized affidavits are requested where the provider offers them.

## What happens when a provider sends an incomplete record?

Every delivery is checked against what was requested. Missing pages or date ranges go back to the provider automatically, and a provider that has no records on file is documented as such on the request.

## Can we rush a request for a deposition or trial?

Put the deposition or trial date on the case. Requests on that case move to the front of the work queue and get priority follow-up. There is no separate expedite fee.

## Which record types can Tavrn request?

Medical records (full chart, ER, mental health, radiology reports), billing (physician, facility, ER, radiology, anesthesia, ambulance), imaging and films, pharmacy, accident reports and insurance coverage. Medicare, Medicaid and insurer claim files are not supported today.

## How do provider fees show up in our accounting?

Provider fees and Tavrn fees post to the matter as native expenses in Litify, SmartAdvocate or Clio, split by type with a recoverable flag, and the invoice PDF is filed with the records. QuickBooks Online receives the same fees as bills or purchases.

# Outside records, with the images and slides

Tavrn requests notes, images and pathology slides from each provider a patient has seen, on one HIPAA authorization, and HIM files them to the chart.

**100,000+** provider requests · **425,000** provider organizations in Tavrn’s directory · **Every 48 hours** follow-up with each open provider

## The outside chart decides the plan

- **52%** of the information clinicians lacked at the visit sat outside the practice: the notes, labs and imaging reports.
- **42%** of repeat CTs after trauma transfer were ordered because the outside scan was unavailable.
- **Up to 67%** of admission medication histories contain at least one error. The prescriber’s list and dose changes sit in the outside chart.

Tavrn requests all three from the providers that hold them.

## Where the outside chart is read first

Service lines that plan treatment from records written elsewhere.

### Oncology

Confirm stage before tumor board. Records: Slides and blocks, DICOM images, Treatment doses. Without them: Prior doses go uncounted.

### Fertility

Choose the stimulation protocol. Records: Stimulation sheets, Embryology reports, Hormone and semen tests. Without them: Dosing starts over; the payer sees no prior cycle.

### Transplant evaluation

Clear the candidate for listing. Records: Dialysis records, Psychosocial evaluation, Blood type. Without them: Listing waits.

### Neurology

Confirm the referred diagnosis. Records: MRI as DICOM, EEG and spinal fluid. Without them: Repeat the MRI, or accept the referral’s diagnosis.

### Structural heart

Choose catheter valve or surgery. Records: Echo loops, Gated CT, Cath films. Without them: The valve decision waits on repeat imaging.

### Behavioral health & SUD

Set the medication plan. Records: Discharge summaries, Medication trials, Methadone doses. Without them: Failed medication trials restart.

## One signature to one folder

### Authorize

The patient signs one HIPAA authorization. Tavrn prepares each provider’s own release form from that signature.

### Request

Tavrn asks networks and portals first for what they hold, then faxes, mails, emails or calls the providers networks do not reach.

Patient visits by route: **63%** networks, records vendors, portals; **37%** requested directly. Networks carry documents, rarely images; never slides, blocks, paper charts or bills.

### Follow up

Tavrn contacts each open provider every 48 hours until the records arrive or the provider confirms there are none.

### Check

Tavrn compares deliveries with the request: dates of service, record types and pages. A chart that stops early goes back to the provider.

### Deliver

HIM receives one folder per patient, by provider and date of service, in the Tavrn app, SFTP or API.

Provider fees are passed through at exact cost.

## How hospitals and clinics use Tavrn

Who opens the request, and what moves when the records arrive.

- **New-patient intake** — Patient access requests the outside records at booking, from each provider the patient names.
- **Referral review** — The referral coordinator closes the consult with the outside result filed, not a note that it was requested.
- **Tumor board** — The tumor-board coordinator lists the case with outside slides, blocks and DICOM in hand.
- **Transplant evaluation** — The transplant coordinator takes dialysis run sheets, echo loops and prior operative notes to committee.
- **Transfer center** — When the outside CT arrives as DICOM, the accepting team reads the images instead of repeating the scan.
- **Prior authorization** — The authorization specialist attaches the dated outside physical therapy notes the payer requires.

### What comes back

- Clinical notes
- Imaging reports and DICOM images
- Pathology reports, slides and blocks
- Labs
- Operative notes
- Medication and infusion records
- Itemized bills
- Scanned and legacy charts
- Behavioral-health and SUD records, under Part 2 consent

## Eligibility decided from documents, not codes

### Documents written at other centers

Prior treatment regimens, biomarker reports, baseline scans and organ-function labs settle a match. Most were written at the diagnosing center and the treating practice.

### Requested on the research authorization

Tavrn requests pathology, imaging and infusion records from each provider on the patient’s signed research authorization, valid to the study’s end.

## Before the first request

## Do we get the images and slides, or only the reports?

Both. Imaging comes as DICOM with its report from the center that holds it. Slides or blocks come from the lab; no network carries them. The receiving pathologist reviews the original slides.

## Which providers can Tavrn request outside records from?

Any US provider the patient has seen: hospitals, independent practices, imaging centers, labs and behavioral-health programs, about 425,000 organizations in Tavrn’s directory.

## How long does it take to get records back?

The provider’s clock; a release on an authorization has no federal deadline. Tavrn follows up every 48 hours and shows each provider’s status throughout.

## What can providers charge for copies of records?

Requests under an authorization follow state fee schedules. Each provider’s invoice appears on its own line of the request.

## Can Tavrn retrieve behavioral-health and substance-use records?

Yes, on the patient’s consent. Substance-use program records require a written consent that meets 42 CFR Part 2, naming the program, the recipient, the information and the purpose. Tavrn requests them from the program directly.

## Can our research office use Tavrn for trial eligibility records?

Yes, on the candidate’s HIPAA research authorization, which may run until the study ends. Tavrn requests pathology, imaging and infusion records from each provider directly.

## How do records reach our EHR or document system?

In the Tavrn app, by SFTP into the document system, or through the API with a webhook on every status change. HIM files it to the chart.

## Will Tavrn sign a BAA, and is there a SOC 2 report?

A Business Associate Agreement is available on request and reviewed during the security review. SOC 2 Type 1 and Type 2 reports and a HIPAA attestation are shared through the trust center on request.

Tavrn handles protected health information under HIPAA. Records are encrypted at rest and in transit, and every outreach attempt on a request is logged. Details on the trust center.

# Medical record retrieval as an API

The patient signs once. Imaging, pathology slides and charts are requested from each provider they have seen; a webhook fires on every status change.

**100,000+** provider requests · **425,000** provider organizations in Tavrn’s directory · **Every 48 hours** follow-up with each open provider

## What comes back from every provider

Network connectors return what a site publishes: C-CDA, FHIR, some PDFs. They rarely carry DICOM and do not carry slides, infusion flowsheets or bills.

Tavrn requests each record type from the provider that holds it, whether or not that provider is on a network.

- Clinical and consult notes
- DICOM images with the report
- Pathology reports, slides and blocks
- Labs from every lab and hospital
- Infusion and medication records
- Operative notes
- Itemized bills
- Scanned and legacy charts

## What you build on outside records

Four products that start when the outside chart arrives.

### Care navigation and second opinion

You build: Expert review after a serious diagnosis. Records: Slides, DICOM, Infusion records, Consult notes. What changes: The specialist reviews the original slides and images, not a summary of them.

### Clinical AI and chart review

You build: Chart review across every provider’s records. Records: Scanned charts, Faxed notes, Labs. What changes: The model reads the faxed and scanned pages too.

### Virtual specialty care

You build: Specialist care that begins by video. Records: Prior labs, DICOM, Medication records, Operative notes. What changes: Network treatment queries require a treating provider. Your request runs on the patient’s authorization alone.

### Trial matching and recruitment

You build: Eligibility screening from the outside documents. Records: Prior regimens, Pathology and biomarkers, Baseline scans, Organ-function labs. What changes: Each criterion is settled from the outside documents.

## One request per provider, status by webhook

Create a request per provider under the patient’s case, by API or spreadsheet, with the signed authorization attached. One authorization covers every US provider the patient has seen.

### One request per provider

Each request carries its own status, contact dates and any provider fee, passed through at cost under the approval threshold you set.

### Networks, then direct

Networks, records vendors and portals return about 63% of a patient’s visits; Tavrn requests the rest from each provider by fax, mail, email and phone.

### Follow-up until answered

Every open provider is contacted every 48 hours until it answers. A webhook fires on every status change, or poll the request.

### Delivery as providers respond

Records arrive as each provider responds. Download them with the same key, or take delivery by SFTP or in the Tavrn app.

Create a request:

```http
POST /v2/cases/{case_id}/retrieval-requests
{
    }
  ],
  "auth_form": {
    "upload_id": "upl_8f2…"
  }
}
```

## Research on the patient’s authorization

Eligibility and long-term follow-up turn on outside documents: prior regimens and stop dates, pathology and biomarkers, baseline scans.

- **Authorization** — Research is not a named network purpose, so your requests run on the patient’s HIPAA research authorization, valid to the study’s end.
- **Cohort orders** — One request per cohort, one row per patient per provider.
- **Source documents** — Each record is the provider’s own chart, delivered as the provider produced it.

## Questions before you integrate

What a request returns, who may receive it, what it costs, and how it reaches your product.

## Do we get the images and slides, or only the reports?

Both are requested. Tavrn requests the imaging report and the DICOM study from the imaging center that holds it, and the pathology report, with slides or blocks, from the lab for your pathologist’s re-read.

## Do we need a treatment relationship with the patient?

No. Records are released to you under the patient’s signed HIPAA authorization, which names your organization and the purpose of use. Research and navigation requests do not depend on a treatment relationship.

## Can we request records for research rather than treatment?

Yes, when the patient’s authorization describes the study; it may run to the study’s end and combine with informed consent. Research is not among the reasons the national networks list for sharing records; your request runs on the patient’s authorization.

## How long does it take to get records back?

Each provider’s own clock. Tavrn follows up every 48 hours and shows each provider’s status on the request.

## What do providers charge for records, and who pays them?

Fees follow each state’s schedule. Tavrn pays each invoice and passes it through at cost.

## How do records and status updates reach our product?

Your product can order by API or by spreadsheet upload, get a webhook on every status change, and download the records with the same key or by SFTP.

## Can you request records from substance-use treatment programs?

Yes, on the patient’s written consent. A federally assisted substance-use program releases records to a third party on a consent that meets 42 CFR Part 2, naming the program, recipient, information and purpose. Tavrn requests them on that consent.

## Will Tavrn sign a BAA with our company?

A Business Associate Agreement is available on request and is reviewed with your team as part of the security review, alongside the documents on the trust center.

SOC 2 Type 1 and Type 2 reports, a HIPAA attestation, and Tavrn’s security policies are on the trust center, shared once a request is approved. More on the security page.

# Medical record retrieval, as an API

One authorization in, complete record out. Create a case, request records from any US provider, get a webhook on every status change, and download structured documents into your product or agent.

## Get complete records without waiting weeks.

### Retrieval as an endpoint for your product or agent

REST API: a webhook fires on every status change. Plain JSON, SDK-free, documented.

### Fees you can see and control

Provider fees passed through at exact cost, under an approval threshold you set.

- **Automated follow-up** — Every open request is followed up automatically every 48 hours.
- **Missing records agent** — Checks every delivery against the request and goes back to the provider for anything missing.
- **Live status on every provider** — Per-provider status, provider-stated dates, and full history on every request

## FAQ

## How do I get an API key?

Test keys are self-serve: sign up at sandbox.tavrn.ai with a work email and get a tavrn_sk_test_ key for synthetic data in a minute. Production keys are issued to customer organizations in the Tavrn dashboard after a 30-minute call. The sandbox never touches production systems or protected health information; the call covers your use case, volumes and the BAA.

## How does authentication work?

Send your key as a Bearer token on every request. Keys start with tavrn_sk_live_ and are scoped to your organization; rotate them from the dashboard.

## What does the patient have to sign?

One HIPAA authorization per patient. Upload the signed PDF through the sign-upload endpoint and reference it as auth_form on each retrieval request; Tavrn handles every provider’s own forms from there.

## How do I know when records are ready?

Subscribe a webhook (POST /v2/webhooks) and a signed delivery fires on every status change, or poll GET /v2/retrieval-requests/{id}: received → processing → completed, with action_required when the team needs something from you.

## Which record types can I request?

Medical, billing and imaging lines on one request: medical_records, medical_records_er, medical_records_mental_health, abstract_medical_records, radiology_reports, rfc and the other 11 types in the reference, plus custom_* with your own label.

## Can an AI agent use Tavrn on my behalf?

Yes. An agent can read agents.md, answer your questions from it, test against the sandbox with a key you create there, and book the call for you. Live record requests still need your organization’s production key and the patient’s signed authorization.

## What types of records can Tavrn retrieve?

Medical records, billing records, radiology images, accident reports and insurance coverage from any US provider.

# Build on the records API

One REST contract for medical record retrieval from any US provider: create a case, attach the signed authorization, request records, get a webhook on every status change. Start on synthetic data today with a self-serve sandbox key.

[Get a sandbox key](https://sandbox.tavrn.ai) · [API reference](https://docs.tavrn.ai/api-reference/v2)

## First request in four steps.

Test keys are self-serve: sign up at sandbox.tavrn.ai with a work email and get a tavrn_sk_test_ key for synthetic data in a minute. Production keys are issued to customer organizations in the Tavrn dashboard after a 30-minute call.

1. **Sign up** — Create a sandbox account at sandbox.tavrn.ai with a work email. No call, no contract, no production data.
2. **Get a key** — Copy your tavrn_sk_test_ key from the key page and export it as TAVRN_API_KEY; every request to sandbox.tavrn.ai/v2 sends it as Authorization: Bearer.
3. **Create a case, request records** — POST /v2/cases, upload a synthetic authorization through sign-upload, then POST a retrieval request with X-Tavrn-Sandbox-Scenario: complete to force the happy path.
4. **Watch it complete** — Register a webhook and receive retrieval_request.status_changed, or poll GET /v2/retrieval-requests/{id} until status is completed.

## Sandbox quickstart

### 1 · Create a case

```
# Paste the key from the sandbox key page once per shell
export TAVRN_API_KEY="tavrn_sk_test_…"
# Create the case the records belong to
curl -X POST https://sandbox.tavrn.ai/v2/cases \
  -H "Authorization: Bearer $TAVRN_API_KEY" \
  -H "Content-Type: application/json" \
  -d '{ "title": "Doe v. Example Motors", "client_name": "Jane Doe",
        "client_date_of_birth": "1980-01-15" }'
Base URL https://sandbox.tavrn.ai/v2 · tavrn_sk_test_ keys only · same endpoints, stages and webhook signatures as production · API reference · OpenAPI. Scenarios: complete, action_required_payment, contact_missing, request_issue, no_records, stalled (append ,slow for realistic timing).
## Four resources. Plain REST.
Every record request lives under a case. Statuses are coarse and public; documents are downloaded with the same key.
- `POST /v2/cases` — A case to attach retrieval requests and documents to
- `POST /v2/cases/{case_id}/sign-upload` — Presigned URL for the authorization PDF or attachments
- `POST /v2/cases/{case_id}/retrieval-requests` — id, order_number, status
- `GET /v2/retrieval-requests/{id}` — Status, timeline, action requests
- `POST /v2/retrieval-requests/{id}/action-response` — Answer a question from the team
- `GET /v2/retrieval-requests/{id}/documents` — Retrieved files, each with a download URL
- `POST /v2/webhooks` — Signed deliveries on every status change
## Request status
- **received** — Accepted; the team is validating the authorization and provider.
- **processing** — Being worked with the provider; follow-ups every 48 hours.
- **action_required** — Tavrn needs something from you — answer via the action-response endpoint.
- **completed** — Terminal. Documents are ready to list and download.
- **cancelled** — Terminal. Cancelled by you or because the provider cannot fulfil it.
## Three events, signed on every delivery.
Register an HTTPS endpoint with POST /v2/webhooks and pick the events you need. Each delivery is signed so you can verify it came from Tavrn; retries follow the schedule in the reference.
- `retrieval_request.status_changed` — Any status or stage change on a retrieval request.
- `retrieval_request.action_required` — The team needs something from you; answer via action-response.
- `retrieval_request.documents_available` — Retrieved files are ready to list and download.
## Same contract. Two things only a human does.
Nothing in your integration changes between sandbox and production except the base URL and the key.
### Sign the BAA
Tavrn handles protected health information under HIPAA. A Business Associate Agreement is reviewed with your team on a 30-minute call before any live request.
### Get a live key
Production keys (tavrn_sk_live_) are created by an admin of your organization in the Tavrn dashboard; they are never self-serve and never work against the sandbox.
### Switch the base URL
Point the same code at enterprise.tavrn.ai/v2, drop the scenario header, and send real signed authorizations. Endpoints, statuses, webhook payloads and error shapes are identical.
[Visit our Trust Center](https://app.vanta.com/tavrn/trust/0tb5amch52699x97ykvhnb)
## Security by design
### Built for protected health information
Tavrn handles protected health information under HIPAA. A Business Associate Agreement is available on request and is reviewed with your team as part of the security review.
### Encrypted at rest and in transit
Records are encrypted at rest and in transit. They reach your team in the Tavrn app, over SFTP or through the API, whichever you choose.
### Access through your accounts
Your team signs in to Tavrn with Microsoft or Google single sign-on, so access follows your organization’s own accounts.
### Every step on the record
Every call, fax, email, mail and portal attempt Tavrn makes on a request is logged with its date, purpose and outcome. Ask for the Outreach Activity Report when you need the full history of a request.
### Independently verified
Third-party auditors assess Tavrn’s controls. Our SOC 2 Type 1 and Type 2 reports and HIPAA attestation are on our Trust Center, shared once your access request is approved.
## What to know before you integrate
The short version for engineering and security reviews. The full documents are on the Trust Center.
### HIPAA and a BAA
Production API traffic carries protected health information under HIPAA. A Business Associate Agreement is available on request and is signed before a live API key is issued.
### Data residency: AWS us-east-1
The Enterprise API and the records it retrieves run in AWS us-east-1 (N. Virginia), encrypted at rest and in transit.
[API reference](https://docs.tavrn.ai/api-reference/v2)
### Subprocessors
The current list of subprocessors is available on request as part of the security review, alongside the SOC 2 reports and HIPAA attestation.
### Sandbox is synthetic-only
sandbox.tavrn.ai runs in a separate AWS account with fictional facilities and patients. It never holds PHI, so you can integrate and run tests before any agreement is in place.
# How Landver Law Personal Injury Attorneys increased demand output 6x with Tavrn
Landver Law Personal Injury Attorneys was struggling with slow, expensive medical record retrieval that delayed demands, settlements, and client payouts.
- **Practice area:** Personal injury
- **Products used:** Medical record retrieval
## Impact
Landver Law Personal Injury Attorneys reported a 6x increase in demand output after implementing Tavrn.
- **6x** — Increase in demands sent
- **Faster settlements** — With faster access to records and bills, many minimum-policy cases now settle in just 2–3 months.
- **Lower client costs** — By reducing retrieval expenses, clients retain more of their settlement proceeds.
- **Increased operational efficiency** — Before Tavrn, staff spent hours: • Following up on records • Troubleshooting retrieval issues • Managing vendor communication  Tavrn eliminated a major operational bottleneck and accelerated case throughput across the firm.
## The Challenge
Landver Law Personal Injury Attorneys was struggling with slow, expensive medical record retrieval that delayed demands, settlements, and client payouts.
Before Tavrn, the firm used multiple retrieval vendors that required constant follow-up, poor communication management, and high retrieval costs. In many cases, staff had to manually chase providers and vendors to identify missing information or stalled requests.
The issue became even more costly because the firm routinely retrieves records from:
• Multiple hospitals and physicians
• Prior providers
• Historical treatment records for exacerbation claims
These delays slowed down the entire case lifecycle and negatively impacted both firm efficiency and client experience.
## The Solution
Landver Law Personal Injury Attorneys implemented Tavrn Medical Record Retrieval to streamline retrieval workflows and accelerate case movement.
With Tavrn, the firm gained:
• Faster retrieval turnaround times
• Lower retrieval costs
• Improved communication and visibility
• Reduced administrative burden
• Faster demand preparation
According to Alina Landver, Esq., MBA, Tavrn’s pricing was approximately 70% lower than the firm’s previous retrieval vendor.
Meet our Client at www.LandverPersonalInjury.com
## Why Tavrn
- **70% Lower Retrieval Costs** — Compared to the firm's previous vendor
- **Faster Client Payouts** — Cases settle faster when records arrive faster.
- **6x Faster Demand Output** — Enabled significantly faster case movement.
- **Better Client Experience** — Clients receive quicker resolutions with lower case expenses.
## The Result
> “If I’m getting six times the amount of demands out, that means I have the records. That means cases are moving.”
> "Tavrn has completely changed the speed at which we can move cases. We’re getting demands out 6x faster because we’re no longer stuck waiting on records. Faster records mean faster settlements, happier clients, and better outcomes for everyone involved."
# How Martay Law Office increased case filings by 65% with Tavrn
By automating medical chronologies and record review, Martay Law Office removed a critical operational bottleneck after a clerk responsible for medical summaries departed the firm.
- **Location:** Illinois
- **Practice area:** Workers' compensation
- **Products used:** Medical record retrieval, Medical chronologies
## Impact
By automating medical chronologies and record review, Martay Law Office removed a critical operational bottleneck after a clerk responsible for medical summaries departed the firm.
- **65%** — Increase in case filings, year-over-year
- **22K+ pages** — Thousands of pages processed in minutes, including records exceeding 22,000 pages
- **Weeks to min** — Weeks of manual work reduced to minutes through AI-powered summarization and querying
- **No backfill** — Eliminated the need to backfill a full-time clerk and paralegal workflow for medical record review
- **+ Cases** — Enabled attorneys to personally handle significantly more cases without increasing headcount
- **Faster insights** — Accelerated identification of key medical facts, including initial history, causation statements, and MMI dates
## The Challenge
Martay Law Office, a high-volume workers' compensation firm in Illinois, faced a significant operational hurdle when a clerk of 12 years — who was responsible for medical chronologies — departed the firm. The firm’s existing workflow required a dedicated person for record retrieval and a full-time paralegal to manually tag history, objective findings, and surgical reports. Without this resource, the firm needed a solution that could handle massive volumes of medical records — sometimes exceeding 22,000 pages — without sacrificing accuracy or incurring the high costs of alternative legal AI platforms.
## The Solution
The firm adopted Tavrn to automate their medical chronology and case review process.
By leveraging Tavrn, Martay Law Office can now upload thousands of pages and receive intelligent, relevant summaries in minutes. Tavrn allows the firm to quickly identify initial history, causation statements, and MMI dates. Additionally, the platform’s interactive AI enables attorneys to ask complex questions directly to the medical records, receiving precise answers that could have previously taken a human clerk a week or more to process.
## Why Tavrn
Martay Law Office chose Tavrn over general AI tools like ChatGPT and other legal platforms for several key reasons:
- **Security & Compliance** — Tavrn provides a secure, HIPAA-compliant environment essential for sensitive medical data.
- **Speed & Accuracy** — The platform’s ability to summarize hundreds of pages quickly allowed the firm to move more cases forward faster.
- **Cost-Effectiveness** — Tavrn provides transparent and simple pricing that is the most cost-effective in the market.
## The Result
Higher attorney throughput, faster case movement, and sustained growth without the cost or risk of additional staffing.
> "The record retrieval and summarization of a couple hundred pages of medical records was unbelievable and crazy fast. Since implementing Tavrn, my filings are up almost 65% year-over-year because I can personally handle way more cases."
# How Paul LLP streamlined record retrieval with Tavrn
The impact was immediate. What previously required multiple full days of paralegal time has been reduced to approximately one to two hours per week, depending on the number of plaintiffs involved. Key results include:
- **Location:** Kansas City
- **Practice area:** Complex litigation and mass torts
- **Products used:** Medical record retrieval
## Impact
The impact was immediate. What previously required multiple full days of paralegal time has been reduced to approximately one to two hours per week, depending on the number of plaintiffs involved.
Key results include:
• A dramatic reduction in time spent on medical record requests
• Faster retrieval across large, multi-facility cases
• Significant relief from administrative burden for paralegals
• More time allocated to higher-value, case-specific work
## About the company
Paul LLP is a Kansas City–based law firm specializing in complex litigation, including class actions, mass torts, toxic torts, antitrust, and product liability cases. These matters often involve large plaintiff groups and medical records sourced from hundreds of facilities, making speed, accuracy, and compliance essential.
## The Challenge
Before adopting Tavrn, medical record retrieval was one of the most time-consuming operational bottlenecks at Paul LLP.
Paralegals were responsible for preparing and submitting medical record requests, identifying the correct facilities and departments, following up on missing or incorrect information, and restarting requests when details were incomplete or inaccurate. On product liability and toxic tort cases, this work often required full eight-hour days, sometimes multiple days in a row, just to get initial record requests out.
On large matters involving thousands of plaintiffs, even small inefficiencies compounded quickly. The process was manual, repetitive, and frustrating, pulling experienced paralegals away from higher-value legal work.
At the same time, Paul LLP is cautious about adopting new technology—particularly AI, due to ethical obligations and data privacy requirements. Any solution needed to be HIPAA-compliant, secure, and purpose-built for legal workflows, not a general automation tool.
## The Solution
Paul LLP adopted Tavrn to centralize and streamline medical record retrieval while maintaining strict compliance and oversight.
With Tavrn, the firm can:
• Initiate medical record requests through a HIPAA-compliant workflow
• Eliminate manual facility research and repeated follow-ups
• Track record requests without relying on spreadsheets, emails, or phone calls
## Why Tavrn
Paul LLP chose Tavrn over general AI tools and traditional record retrieval vendors for several reasons:
- **Security & Compliance** — HIPAA-compliant workflows designed specifically for legal and medical data
- **Speed & Reliability** — Faster record retrieval than internal manual processes
- **Operational Leverage** — Reduces low-value administrative work without adding headcount
- **Built for Complex Litigation** — One of the few platforms capable of supporting large-scale, specialized litigation workflows.
> "We love Tavrn. It’s saved our team an incredible amount of time and taken one of the most tedious, frustrating parts of our workflow off our plate. For firms that need to request records at scale, it’s a game changer."
>
> — Michaela Cussen, Director of Operations, Paul LLP
# Rebuilding how medical records move.
Tavrn retrieves medical records from any provider in the United States for the law firms, health systems, research sites and health-tech companies that depend on them, then turns the records into the work product they need. We are a small team in San Francisco, working in person.
## What we do
A medical record is the starting point for a legal case, a clinical decision, a research enrollment and an underwriting file. Getting it has meant fax machines, phone queues and weeks of waiting. Tavrn does the retrieval end to end, from any US provider, and delivers the record as the work product each customer needs.
- **500+** organizations, from solo practices to national systems
- **425,000** provider organizations reachable, all 50 states and DC
- **100%** of providers chased until the record is in
## How we work
Six commitments we hold each other to. They decide who we hire and how we make calls when the answer is not obvious.
- 01 **Customers first** — Firms and health systems trust us with protected health information and with deadlines that decide cases and care. Every decision starts with what gets the right record to them sooner.
- 02 **Ownership** — Nobody waits for permission. You see the problem, you own the fix, and you change the system so it does not come back.
- 03 **High standards** — A chronology with one wrong date is a wrong chronology. We care about precision, craft, and getting measurably better every week.
- 04 **Direct** — Clear, honest feedback, given early and in person. Low politics and low egos; strong opinions, held loosely.
- 05 **Momentum** — We move quickly, not recklessly. Ship, measure, correct. The best idea wins regardless of title.
- 06 **In the room** — We work from one office in San Francisco. Engineers sit with customers, sales works with marketing, operations is in the field. Problems get solved at the whiteboard.
# One authorization in. The complete record out.
Tavrn retrieves medical records from any provider in the United States for the law firms, health systems, research sites and health-tech companies that depend on them, then turns the records into the work product they need. We are a small team in San Francisco, working in person.
- **500+** organizations, from solo practices to national systems
- **425,000** provider organizations reachable, all 50 states and DC
- **100%** of providers chased until the record is in
## A single process for medical record retrieval
One signed HIPAA authorization covers every provider on the request. Tavrn prepares each release, works the provider by fax, portal, mail, email or phone, follows up every 48 hours and checks every file against the request.
- 1. **Request** — A new request starts in the Tavrn platform, from Clio, SharePoint, Smokeball, Litify, SmartAdvocate, Dropbox, or from the API.
- 2. **Track** — A status card with one row per provider: records received, confirming receipt with a provider-stated date, action required to approve a fee, and the next follow-up in 48 hours.
- 3. **Deliver** — A folder of checked documents filed by provider and date of service, with record type, page count and file type for each file.
## Four kinds of organization, one retrieval process
- [Law firms](https://www.tavrn.ai/solutions/law-firms): Medical record retrieval for law firms: records and bills from every provider, followed up until they arrive and filed into your case management system.
- [Hospitals & clinics](https://www.tavrn.ai/solutions/hospitals-clinics): Medical record retrieval for hospitals and clinics: outside notes, DICOM, pathology slides and legacy charts from every US provider, on one authorization.
- [Health tech](https://www.tavrn.ai/solutions/health-tech): Medical record retrieval API for health tech: imaging, pathology and charts from each provider on one authorization; a webhook on every status change.
