Medical record review

Medical record review services for attorneys

Tavrn reviews a client’s medical records for law firms: every page retrieved from the providers on the matter, checked for completeness, and turned into a cited medical chronology with dated entries by provider, each linked to its source page. Your team reviews the chronology, not the stack, and exports it for the demand.

What the review covers

The complete record first

A review is only as good as the records behind it. Tavrn retrieves treatment records and itemized bills from every provider on the matter on one HIPAA authorization, followed up every 48 hours until they arrive.

Gaps found before review

Every delivery is checked against the request: pages, date range and record types. Missing pages go back to the provider; a provider with nothing on file returns a documented no-records statement.

A cited chronology

Each date of service becomes an entry with the provider, a short summary of the visit and the source pages it came from, so every line can be checked against the record.

Reviewed before it reaches you

The chronology and the demand drafted on your firm’s template are reviewed before they reach you. Your attorneys still decide what the record means for the case.

Continued care

New dates of service are re-requested from the same provider, duplicates detected, and the chronology picks up the new visits.

Works with your tools

Records retrieved through Tavrn can be used with your existing review tools, or with Tavrn’s own chronologies and demands, filed into Litify, SmartAdvocate, Clio or Smokeball.

Review by case type

What a legal record review looks for

The reading changes with the practice area; the method does not: complete records, every entry dated and sourced, gaps on the record.

Case typeWhat the review tracesRecords it depends on
Personal injuryTreatment from the first visit after the incident through the last, and the bills for eachER and hospital charts, imaging, therapy notes, itemized bills
Medical malpracticeThe course of care before and after the event, visit by visitFull charts from each treating provider, operative and nursing notes
Workers’ compensationInitial history, work restrictions and maximum medical improvementTreating-physician notes, therapy records, independent exam reports
Mass tortExposure, diagnosis and treatment across a docket of claimantsRecords and pharmacy history for each claimant, requested in bulk

Review, summary or chronology

One record, three deliverables

A medical record review reads the whole record for the case. A chronology lays it out date by date with sources; a summary condenses it into the facts the case turns on. Tavrn builds the cited chronology from the retrieved record, and the summary from the chronology.

FAQ

Medical record review, answered

What is a medical record review for attorneys?
A legal medical record review reads a client’s medical records for what the case needs: the treatment, its dates and providers, and the bills behind each visit. The usual output is a medical chronology, with each entry dated and tied to its source page, plus a summary of the facts the case turns on.
What does Tavrn’s medical record review include?
Retrieval of the complete record from every provider on the matter, a completeness check against each request, and a cited medical chronology with dated service summaries and source-page references, reviewed before it reaches you and exported for the demand.
Does Tavrn replace the attorney’s or a medical expert’s review?
No. Tavrn organises the record so your team reads a cited chronology instead of the full stack. Legal judgment stays with your attorneys, and medical opinions come from the experts you retain.
What if records are missing when the review starts?
Every delivery is checked for pages, date range and record types before review. Missing pages go back to the provider on the same request, and a provider with nothing on file returns a documented no-records statement, so the gap is on the record.
We already use an AI tool for record review. Is Tavrn redundant?
No. Those tools start when the records are in hand; Tavrn is how they get there. Records retrieved through Tavrn can be used with your existing tools, or with Tavrn’s own chronologies and demands.
Which practice areas is it for?
Personal injury, mass tort, medical malpractice and workers’ comp firms. Mass tort dockets use bulk ordering and the API.
Where does the reviewed record go?
Into the matter in Litify, SmartAdvocate, Clio or Smokeball, or SharePoint, Dropbox, the Tavrn platform or the API, with the records searchable (OCR), with a cover page, organised by provider and date of service.
